Subintimal tracking and re-entry technique for stent-jailed side-branch occlusion

The SB-STAR technique, a novel treatment for side branch occlusion after stenting of the main trunk of a coronary artery bifurcation, may be an effective therapeutic strategy for resolving side branch occlusion, Dr. Yusuke Morita of the Sapporo Cardio Vascular Clinic and colleagues reported in the online edition of the Journal of Invasive Cardiology.

Dr. Morita and colleagues studied the clinical outcomes and usefulness of this technique in 10 of 13,421 patients who underwent elective PCI from January 2016 to June 2021, at a single center.

The mean age of the patients was 68.1 years, the proportion of male patients was 40%, and the target lesions were the LAD and the bifurcation of the diagonal branch in 9 cases and the LCX and the OM in 1 case. The guidewires used for the SB-STAR technique were polymer jacket in 7 cases and floppy in 3 cases, and stenting to the side branches was performed in 3 cases.

Seven patients achieved final TIMI flow 3 and the rest achieved TIMI flow 2, recording a 100% of the procedural success. Procedure-related MI was observed in 4 patients, and in-hospital death, clinical tamponade, and emergency TVR were none.

After 6 months of follow-up, there were no deaths or TVR, and all patients underwent contrast and patency was confirmed in 8 patients (80%). No patency was observed in one patient treated with LCX and the other with LAD. There were no new wall motion abnormalities or LVEF reductions of more than 10%.
